ICS Kid's Art Festival Registration Form

Registration and participation is free. Participants to the art atelier are requested to register via online registration form below. Please fill it out as completely as you can, choose the desired date and time slot then click the "Register" button at the bottom of the page. Confirmation will be sent by e-mail to all participants upon receipt of their online registration. Participants should bring this confirmation with them on the date of their choice. Only registered participants and their parents/guardians will be admitted to the art atelier workshops.

Deadline for registration is September, 11th 2009.
Please kindly ensure to register by this date.

Although the art atelier spans over two days, please note that the number of participants is limited due to space constrains. It is recommended to register as soon as possible to ensure a space is allocated to each participant. Bookings will be made on a first-come, first-served basis.

(hereinafter referred to as ICS) with the child named above for as long as he/she stays on the premises of ICS to produce the artwork within the framework of ICS Kid's Art Festival 12-13 September 2009. If I do not stay, I undertake to be contactable for the duration of the art atelier workshop session attended by the child named above and I will return to ICS to pick him/her up at the end of the agreed period. As Parent/Guardian of the child named above, I undertake to properly notify ICS staff in the event that someone else must pickup the child. A specific form needs to be filled out for this purpose and can be obtained at the check-in point at ICS. The person picking up the child must show a photo ID at the checkout. As Parent/Guardian of the child named above, I also undertake to inform ICS if the child named above has any allergies (i.e. food, etc.) or dietary restrictions. This information can be specified in the "Message" field of the Online Registration Form or filled out at the check-in. ICS has taken all reasonable efforts to ensure a positive infrastructure within which the children may participate within the ICS Kids Art Festival 2009. As Parent/Guardian of the child name above I hereby release ICS of any responsibility and do not hold ICS or the artists in attendance liable for the child left in ICS's and their care. Copyright: The ICS Kids Art Festival 2009 is free of charge, however the artworks produced by the child named above become and remain ICS's property. As Parent/Guardian, I hereby transfer all rights and ownership, unconditionally and irrevocably of the art work and pieces created by above-named child within the ICS Kids Art Festival 2009. This includes and is not limited to its entry within the competition organised by ICS and its offer for sale within the exhibition of the children's work in ICS's offices (September 2009 - March 2010). Furthermore, said artwork may be reproduced for commercial or non-commercial purposes, any proceeds of which will be donated to a children's charity of ICS's choice. Consent to Use and Publication of Image: I hereby give ICS the right and permission to use of my child's name, for the purpose of reproduction, exhibit, display or publishing, including photographic or video images that might be taken during the ICS Kids Art Festival 2009.
